1. Does script need to be protected by .htaccess in order to work with it?

No, script itself behaves in a way so it requires user to login in order to access it as admin. Technically saying script requires basic authentication by browser. When script is being accessed as a form processor it only checks if form is registered and if so then process its data according to settings. So there is no need to use any external .htaccess file - or form submittin will require user to login into protected area - not what you want, isn't it :) ?
2. I copied form url into form managment area and added new form there, but why the form doesn't work?
2. In order to have form working with AFH script you need to point your form to the script, this procedure is well-described here.
